AllCARE for Seniors

is a Program of All-Inclusive Care for the Elderly (PACE)

PACE is a program of the Commonwealth of Virginia

& Appalachian Agency for Senior Citizens (AASC)

AllCARE for Seniors provides services to participants at home, at the PACE center, and in the hospital.

Our interdisciplinary team of health care professionals includes the following:

· primary care doctor,

· registered nurse,

· therapists,

· Master’s-level social worker,

· pharmacist,

· dietitian,

· home care coordinator,

· transportation coordinator,

· personal care attendants, and

· PACE center manager.

Medicaid, Medicare, and private-pay options are available to AllCARE participants.

Our health care team is available to meet your care needs  24 hours a day, 7 days a week.
